Terms & Conditions - Patient Education and Appointment Request Bot

Please read these Terms & Conditions carefully before using the patient education and appointment request bot provided by Correlate Health's LooPT Chatbot. By accessing and using the chatbot, you agree to be bound by these Terms & Conditions. If you do not agree with any part of these Terms & Conditions, please refrain from using the chatbot.

1. Privacy and Data Collection: a. The chatbot operates in compliance with the Health Insurance Portability and Accountability Act (HIPAA) regulations to ensure the security and privacy of patient information. b. By using the chatbot, you grant Correlate Health permission to receive and collect information regarding your health conditions, treatment history, and other relevant healthcare data necessary to provide accurate patient education and appointment request services. c. Correlate Health takes the privacy of your information seriously and adheres to strict security measures to protect your data. However, no data transmission over the internet can be guaranteed as 100% secure. By using the chatbot, you acknowledge and accept the inherent risks associated with data transmission.

2. Patient Health Information (PHI): a. The information collected through the chatbot may include personal health information (PHI), as defined under HIPAA. b. Correlate Health maintains the confidentiality of PHI and strictly abides by all applicable laws and regulations governing the use, disclosure, and protection of PHI. c. The PHI collected through the chatbot is used solely for the purpose of providing patient education and facilitating appointment requests. It is not shared with any unauthorized third parties without your explicit consent, except as required by law.

3. Limitation of Liability: a. The use of Correlate Health's LooPT Chatbot is at your own risk. The chatbot is provided on an "as is" and "as available" basis without warranties of any kind. b. Correlate Health and its affiliates, partners, and employees shall not be liable for any direct, indirect, incidental, consequential, or special damages arising out of or in connection with the use of the chatbot. c. Correlate Health makes no representations or warranties about the accuracy, reliability, completeness, or timeliness of the information provided by the chatbot. It is your responsibility to verify the accuracy and relevance of any information obtained through the chatbot.

4. Indemnification: By using the chatbot, you agree to indemnify, defend, and hold Correlate Health, its affiliates, partners, and employees harmless from any claims, liabilities, damages, costs, or expenses arising out of or in connection with your use of the chatbot, including but not limited to any violation of these Terms & Conditions.

5. Modifications and Termination: Correlate Health reserves the right to modify or terminate the chatbot at any time, without prior notice. We may also update these Terms & Conditions from time to time. It is your responsibility to review the most current version of the Terms & Conditions periodically.
